APPEARANCE REQUEST FORM

Requests for appearances by a Whitecaps player or other member of the Whitecaps organization must be submitted at least sixty (60) days in advance of the requested date.  Appearances are for a maximum of two hours. 

Confirmation of the appearance will be sent to the listed contact person ten days prior to the event date.

Requests can be made in one of the following three ways:

1) Fill out and submit the online form below
2) Print the form (click here), fill out and fax to 604.684.5173
3) Print the form (click here), and mail to:

Please note that due to game and training schedules, certain days and weekends are unavailable for appearances. 
A high demand for appearances means that unfortunately not all requests can be granted.
